Complete the ONLINE REGISTRATION FORM and PayPal if paying by credit card.

• Please note that the coach names entered on the online registration form will be the two coaches admitted free per registered team at the event.

Submit payment if paying by money order/certified personal, business or school check. All payments must be received prior to the registered event.

Copy Waiver of Liability/Medical/Publicity Release form Download Form and Good Sportsmanship Agreement Download Form (required) for every team member.

• Parents must sign both and competitors must sign sportsmanship form.
• Collect the signed forms from all participants—coaches sign the back of each and turn the forms with original signatures into the coach’s registration table at the event.
• Team members who do not turn in fully completed forms will not be allowed to compete (no refunds).

You need JavaScript enabled to view it at least 4 days before the registered event.  The mp3, wmv or m4p file name should be your Team Name and Division.  Your music will be waiting for you at the sound table.  A confirmation email will be sent that your music was received.  You may also bring a CD, USB drive or electronic media player to the sound table.
